Getting To Know Me

Getting to know me project is a cooperation between Emily Carr university and Haro park nursing house. Every resident have to fill out a form to record personal information when they check in. This form is also a tool to help caregiver understand the resident. Unfortunately the design of the original form is not humanized.

My design of this form was inspired by a CMYK calendar I bought before and Chinese bus ticket’s design. I want to separate each question and classify questions with different color. The same type of question is marked with the same color. For example, the family wise Q is marked red and the hobby is blue. I hope that such a form is no longer just a piece of paper. It can be torn off and reassembled into a small book. This will make it more portable and convenient for nurses or visitors to read the information on the form at any time.
